Desert View Hospital is a 25-bed, acute care hospital set in a rural community. Desert View provides safe and competent nursing care for patients in accordance with facility policies, standards, and philosophy. We provide professional leadership and support to the healthcare team. The concepts of Patient Centered Care will provide the foundation for all nursing care. The Clinical Social Worker will provide essential psychosocial services to patients and their families, helping them navigate emotional, social, and financial challenges associated with medical conditions. This role involves conducting assessments, developing treatment plans, coordinating with healthcare teams, and connecting patients to community resources.

Key Responsibilities:
Conduct psychosocial assessments to determine patients' emotional, social, and financial needs.
Develop and implement individualized treatment and discharge plans in collaboration with the healthcare team.
Provide crisis intervention, counseling, and support to patients and families.
Advocate for patients' rights and access to healthcare services.
Coordinate with community agencies and resources to ensure continuity of care.
Assist patients with advance care planning, including durable power of attorney and end-of-life discussions.
Maintain accurate and timely documentation of patient interactions and treatment plans.
Participate in interdisciplinary team meetings and case discussions.
